professional growth. Job Summary: The speech
pathologist/audiologist CFY is a qualified professional who
evaluates and provides speech therapy services to patients in the
clinic as prescribed by the attending physician. Responsibilities:
Provide assessments and treatment activities with patients within
the parameters of the regulations outlined by ASHA regarding the
Clinical Fellowship Year (CFY) with the appropriate supervision of
a licensed Speech-Language Pathologist Schedule and plan activities
to provides rehabilitative therapy for speech and language
disorders Records type of treatment and patient’s reaction to it on
clinical/progress notes, which are written on the day of the visit
and incorporated in the patient’s record weekly Maintains adequate
records on all patient’s, including summary reports Utilizes
community resources and Agency personnel for proper referral
Selects and administers diagnostic and therapeutic techniques and
materials Instructs and counsels other health team personnel and
family members in methods of assisting patient in improving,
correcting, and accepting his/her disabilities Attends case
conferences as requested Participates in staff development
activities and in-service education as requested Confirms, on a
weekly basis, the scheduling of visits with the Patient Care
Coordinator to coordinate necessary visits with other personnel
Notifies Agency of absences due to illness, emergency leave, normal
vacation periods, or special professional meetings, which will all
affect agreed services with the Agency Participates with staff,
patient, and physician in discharge planning activities and
completes the speech therapy discharge summary within the
designated framework of the Agency policies Requirements:
Qualifications: Master’s degree in Speech-Language Pathology from
an accredited program. Certified member of the American
Speech-Language-Hearing Association (ASHA) or eligible for CFY.
Current Texas license to practice as a Speech-Language Pathologist
(SLP), or eligibility to obtain licensure prior to start date.
Organizational Relationship: The Speech Pathologist/Audiologist CFY
reports directly to the Supervising Speech Pathologist Special
Requirements: Reliable transportation and ability to travel to
patient homes within the assigned service area. Valid Texas
driver’s license and current automobile insurance in accordance
with state requirements. Ability to meet documentation and
compliance standards as outlined by state and federal home health
regulations.
